The optimal treatment is not fully understood, but it is thought to occur in response to physical symptoms or exercise. The most common causes of PAH are high blood pressure in the lungs (hypertension), high blood pressure in the heart (PAH), high blood pressure in the leg (PAHTLE), and a condition called peripheral vascular disease. The symptoms of PAH include chest tightness, wheezing, shortness of breath, and feeling faint or lightheaded. These symptoms are usually short-term and are usually mild and subside with increasing blood pressure.
Tadalafil is an oral PDE5 inhibitor. It works by relaxing the blood vessels in the penis, improving blood flow.
Before we explain how Tadalafil helps manage ED, we need to explain how erections occur naturally. When you become aroused, your body releases a substance called nitric oxide (NO). In turn, levels of a chemical known as cyclic guanosine monophosphate (cGMP) increase in your body, relaxing the muscles on the sides of your penis so blood can flow into the organ and trigger a stiff erection.
While all this is happening, your body also compresses the veins that take blood away from your penis. This increased blood flow causes a slight increase in blood pressure. Eventually, the red blood your body has with tadalafil is used using it to help prevent you getting an extremely low ED risk prostate-related side effect called penile fibrosis (androgens deficiency).

How to Use Tadalafil
As this is a medication, you should always follow the instructions given to you by your doctor or pharmacist. All medications are taken orally with or without food. You should take Tadalafil about 1 hour before sexual activity so that the medication can take this effect for best results.
As this substance is already present in the blood stream it helps increase the levels of cGMP by regulating the flow of cGMP into the penis.
As cyclic alkylates it on the same molecule it releases into your bloodstream cGMP on a separate level called the smooth muscle. When cGMP is phosphorylated on one of the two proteins on the smooth muscle that controls the blood flow into the penis, it creates an erection.
When tadalafil is taken it may help increase the levels of cGMP in the penis. This increase can help keep you able to get an erection even on the very rare occasion when it does not occur.

This condition can be caused in many ways including by taking a medication such as riociguat (Adempas) or by taking any other blood pressure medication. Just because you take a high dose of tadalafil does not mean that you have a medical condition that may be associated with the use of this medication.
If you take riociguat or other other medications for ED, you may also need to discuss with your doctor how Tadalafil may affect your ability to get an erection. This discussion is with your doctor before you start taking any medications.
If you take riociguat or any other medications for ED or have any medical conditions, you should always follow the instructions provided by your doctor or pharmacist. This includes any medications you are taking currently and any herbal supplements you have ever experienced with tadalafil. And always use the lowest effective dose for the shortest possible time.

Tadalafil belongs to the group of medicines called phosphodiesterase type 5 (PDE 5) inhibitors, primarily used to treat erectile dysfunction (impotence) and benign prostatic hyperplasia (BPH). Erectile dysfunction is the inability to keep a hard and erect penis suitable for sexual activity. Benign prostatic hyperplasia, also called prostate enlargement, is the non-cancerous growth of the prostate gland.

Most of these side effects do not require medical attention and will resolve gradually over time. However, you are advised to talk to your doctor if you experience these side effects persistently.
Do not take Tadalafil if you are taking nitrates (used to treat angina), riociguat (used to treat pulmonary hypertension) or alpha-blockers; if you have heart problems or stroke, low blood pressure or uncontrolled high blood pressure. Tadalafil is not intended for use in women. Tadalafil may cause dizziness, so drive with caution. Avoid consuming alcohol as it might cause increased dizziness. Tadalafil should not be given to children as safety and effectiveness have not been established. Keep your doctor informed about your health condition and the medicines you are taking to avoid unpleasant side effects.

1.Nitrates (e.g., nitroglycerin, isosorbide dinitrate): Cialis and nitrates both lower blood pressure. Taking them together can cause a significant and potentially life-threatening drop in blood pressure (hypotension), leading to dizziness, fainting, or even heart attack or stroke. It is contraindicated to use Cialis with nitrates or recreational "poppers" (amyl nitrites).
